Gojii (Self-Management Support)
Gojji is the only provider offering a cellular-enabled
glucometer (no mobile app required) and clinical services from dietitians,
health coaches and pharmacists, at no cost for all Medi-Cal patients. Gojji’s
proven disease management program enables your Community Health Centers to
identify patients needing further care follow-up and to meet quality and
wellness goals. We offer a comprehensive suite of solutions that allows your
health centers to depend on gojji to move your quality needle.
- Testing Kit: cellular-enabled glucometer, blood
glucose test strips, lancets, lancing device, control solution, ketone test
strips (high-risk), and alcohol pads
